Welcome to Prosthodontics of Central Indiana! We are honored that you have chosen our practice for your specialized dental care.

To ensure your first visit with Dr. Abey or Dr. Phasuk is as efficient as possible, we ask that you complete your initial registration paperwork in advance. Sharing your medical history and dental goals ahead of time allows our clinical team to focus entirely on your consultation and the development of your personalized treatment plan.
Please click the links below to download and print your new patient forms. You can fill them out in the comfort of your home and bring the completed copies with you to your scheduled appointment.

 

 

New Patient Checklist

To help us provide the most accurate assessment and ensure a smooth check-in process, please bring the following items to your consultation:

  • Completed New Patient Forms: Your printed and filled-out paperwork.
  • Current Medication List: A complete list of all prescriptions, vitamins, and supplements you are currently taking.
  • Dental Insurance Information: Your current insurance card and any necessary claim forms.
  • Recent X-rays or CBCT Scans: If your referring dentist has recent imaging, please have them emailed to [email protected] or bring a digital copy with you.
  • Photo ID: A valid driver’s license or government-issued identification.
  • Referral Slip: If you were provided a physical referral form by your general dentist.
